Intake Process

Lisa Ann Mikulencak
Initial Contact:

  • Please contact Lisa Ann by telephone: 206.683.8625 or e-mail: LisaAnn@EastLakeUnionCounselors.com. 

  • After talking, you and Lisa Ann will decide if it will be a good match. If you both agree, a first appointment will be set up.
  • You can also start the process by scheduling your first appointment online, either for yourself or your child. There will be an online intake to complete at that point.

Schedule:

  • Lisa Ann works Tuesdays, Wednesdays, Thursdays, and Fridays with morning, afternoon, and early evening appointments.
  • You can schedule and maintain your appointments using her online scheduler.

Forms:

  • Please also read her Disclosure Statement. Download the signature page, sign it, and bring it with you. Thank you. 

Payment and Insurance:

  • Lisa Ann's rate for a 45 minute session is $120. 

  • Please bring your insurance card to your first appointment.

  • Lisa Ann is a preferred provider for First Choice Health, Premera, and Regence. If you have one of these, she will submit the claim to the insurance company. You are responsible for your co-pay.

  • If you have different insurance, Lisa Ann will provide you a detailed receipt that you can submit for reimbursement. Please bring payment with you.

  • Lisa Ann also accepts VISA and Mastercard.

  • All income levels are served. If you are in need of a sliding scale, please give her a call.
