Intake Process

Initial Contact:
- You and I will chat about what you are looking for and what your goals are. If we both think that we will work well together, an appointment will be set up.
Schedule:
- I have office hours on Mondays and Tuesdays in the morning, afternoon, and early evening.
Forms:
- Prior to your first appointment, download and complete my intake form: Intake Form. Please bring the intake form to your first appointment.
- Please read my Disclosure Statement. Print out a copy of the signature page, sign it and bring it with you.
Payment and Insurance:
- My rate for a 50 minute session is $100.
- Please bring your insurance card with you.
- I am a preferred provider for Regence and First Choice Health. I will submit a claim to the insurance company and then bill you for your co-pay.
- If you have different insurance, I will provide you a detailed receipt that you can submit for reimbursement. Please bring payment for your session.
- All income levels are served. Please contact me to discuss a sliding scale.
